Comments:
An unannounced Complaint Inspection was conducted 4/25/22 in response to a complaint that was received by the Virginia Department of Social Services, Licensing Division. Interviews were conducted with staff relating to allegations verbal abuse. Based upon a preponderance of evidence, the information gathered during the investigation does not support the allegations of verbal and physical abuse, and the complaint is determined to be not valid. Based on review, the facility did not ensure that they obtained documentation of children's physical exams within 30 days of the first day of care. Evidence: reviewed 4 children's records and found that 1 (Child 3) did not have documentation of a physical exam.
