Comments:
An unannounced complaint inspection was conducted on 9/9/22 in response to a complaint that was received by the Virginia Department of Social Services, Licensing Division. Interviews were conducted with staff relating to allegations of supervision and procedures for emergencies. Based upon a preponderance of evidence, the information gathered during the investigation supports the allegation of the lack of procedures for emergencies, and the complaint is determined to be valid. Please complete the columns for 'description of action to be taken' and 'date to be corrected' for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office within 10 business days. Based on review, the facility did not ensure that they maintained a written record of serious and minor injuries in which entries are made the day of occurrence. The minimum required information shall be documented. Evidence: reviewed Child 1's record and interviewed staff, and found that they did not document an injury that occurred at the facility.
