Comments:
An unannounced initial subsidy inspection was conducted on November 2 2023 between 12:30pm and 2:00pm. There were 12 children in the direct care of 6 staff members. A review of child record documentation, staff documentation, attendance records, emergency items, evacuation procedures, and the physical plant was conducted. Children were observed participating in music time, hand washing, eating lunch, and preparing for dismissal. Violations are listed on the violation notice page. Based on record review, the center did not ensure that a written allergy care plan, with instructions from a physician, was received for each child with a food allergy.

Based on observation and interview, the vendor did not have a written emergency preparedness plan that address staff responsibility and facility readiness with respect to emergency evacuation, relocation, lockdown, and shelter-in-place procedures.
Evidence: The director stated they do not have a plan written down, but do practice with the children and have locations identified for evacuating and sheltering in place.
