Please complete http://www.dss.virginia.gov/facility/injury_report_subsidy.cgi to report any serious injury to the Division. Serious injuries are defined as an injury sustained while in day care that resulted in the child getting outside medical treatment.

An unannounced Subsidy Health and Safety Inspection was conducted on July 26, 2019 to review supplemental health and safety requirements. A licensing inspection was also conducted on this date (see separate report for details). Only subsidy requirements that are not covered in the licensing standards are reviewed during this supplemental inspection. There were 41 children present ranging in age from 2 months to 12 years old. All staff must have documentation of the VA Pre-service Training and certification in first aid and CPR within the first 90 days of employment. All children with diagnosed food allergies must have an allergy action plan completed by the child's physician. Reminder: evacuation drills are required monthly, shelter in place drills are required twice a year, and a lockdown drill is required once a year. Lockdown drills are to isolate children and staff from a security threat; shelter in place is to protect children from a natural or manmade threat. The last documented lockdown drill was conducted 6/19/2019.
