Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A complaint inspection was initiated on 4/23/2021 and concluded on 05/28/2021. A complaint was received by the department regarding allegations in the area of injuries and incident reports. The director was contacted by telephone to conduct the investigation. The inspector emailed the director a list of documentation required to complete the investigation.
The evidence gathered during the investigation supported the allegations of non-compliance with standards or law.

Complaint related: Yes
Description: Based on staff interview and review of documentation, the facility failed to ensure care, protection, and guidance when supervising children. Evidence: around 3:10 p.m. on 4/16/2021, two toddlers were fighting over a toy. The teacher told the children to "share". Shortly after, the teacher heard one child "cry out" and saw that he had sustained a bite on each cheek and a scratch under the eye from the other child.

Plan of Correction: Staff will receive training on behavioral guidance and toddler interactions.

Complaint related: Yes
Description: Based on staff interview and review of documentation, the facility failed to maintain a complete written record of children's injuries in which entries are made the day of occurrence. Evidence: on 4/16/2021, a parent received a written incident report for an injury, which was only signed by the director and not the parent or a second staff person. The report did not include any future action to prevent recurrence of the injury.

Plan of Correction: Incident reports will be fully completed.
