Comments:
A monitoring inspection was conducted on March 30, 2022 with the center director. There were 38 children present, ranging in ages from 4 months to 5 years, with 7 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on observation, the facility failed to keep all areas inside the center clean and operable. Evidence: the lower level of the building had several ceiling tiles that were broken and had damage, including one directly above the infant room changing table.

Center owner will contact maintenance to replace the tiles.

Description:
Based on observation, the facility failed to keep all hazardous substances such as cleaning materials in a locked place. Evidence: bleach water solution was in an unlocked cabinet under the sink in the downstairs preschool bathroom.

Description:
Based on observation, the facility failed to provide sinks for children with water temperature at 120 degrees or less. Evidence: the sink in preschool room measured at 123 degrees.

The water temperature will be turned down to 120 degrees or less.
