Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated and concluded on July 13th, 2020. The director was contacted by telephone to initiate the inspection. There were 45 children present with 14 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 4 children's records and 4 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and issued to the facility.

Based on a review of children's records, the center failed to obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence: Child #1's first day of attendance was 9/3/2019; however, the child's immunization record was dated 9/5/2019.
Plan of Correction:
We will obtain verification of immunization by the first day of attendance of enrolled child.
