Comments:
An unannounced monitoring inspection was conducted on May 25, 2023 from 9:00 am until 12:00pm. There were 81 children present with 18 teachers supervising. The physical plant, children's records, staff records, bottles, cribs, evacuation logs, attendance logs, medications and authorizations were reviewed. The playgrounds were also inspected. Violations:
Standard #: 8VAC20-780-60-A
Description: Based on a review of children's records, the center failed to obtain all required documentation for each child enrolled.


Evidence:
Child #1, #2. #3, #4 and #5's records do not contain documentation of the names of person authorized to pick up the child.

Plan of Correction: I will get the information from the parent's and place in children's files.

Standard #: 8VAC20-780-280-B
Description: Based on observation the center failed to ensure that hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
There was an unlocked closet that contained chemicals next to the Director's office.

Plan of Correction: Closet was locked during the inspection.

Standard #: 8VAC20-780-280-H
Description: Based on observation the center failed to ensure cosmetics, medications, or other harmful agents shall not be stored in areas, purses or pockets that are accessible to children.

Evidence:
In the Active Asteroids classroom, there was a pocketbook handing on the hook that the children's jackets were on.

Plan of Correction: Teacher's will be reminded to keep personal articles out of reach of children.

Standard #: 8VAC20-780-320-B
Description: Based on observation, the center failed to ensure that restrooms be equipped with soap, toilet paper and disposable towel or an air dryer within reach of the children.

Evidence:
The downstairs hall bathrooms did not contain soap.

Plan of Correction: Soap was put into the bathroom during the inspection.

Standard #: 8VAC20-780-560-G
Description: Based on observation, the center failed to ensure that when food is brought from home, the food container shall be sealed and clearly dated and labeled in a way that identifies the owner.

Evidence:
One bottle in the refrigerator had a name, but was not dated.

Plan of Correction: I will remind parent's to place child's name and date on bottles. I will also remind teachers, that if parent's for get the name and date, they should add them before placing bottle in the refrigerator.
