Comments:
The licensing inspector conducted an unannounced inspection in response to a self-reported incident regarding special services that occurred at the center on July 8, 2019. The department was notified of the incident on July 9, 2019. The investigation was initiated on August 8, 2019 and completed on August 28, 2019. On August 8, 2019, the licensing inspector was on site from 2:30pm to approximately 3:50pm. Interviews were conducted, observations were made in the center, and documentation was reviewed. On August 28, 2019, the licensing inspector reviewed further documentation provided by center. The preponderance of the evidence gathered during the investigation validates the self-reported incident. See the violation notice for more details.

Violations:
Standard #: 22VAC40-665-790-F
Description: Based on a review of documentation and interview on 08/08/2019, the vendor did not ensure that a child with a diagnosed food allergy shall not be served any food identified in the written care plan required in 22VAC40-65-520 B 11.

Evidence:
1) The written care plan in the record for Child #1 (two-year-old) indicates the child has a diagnosed food allergy to wheat and eggs.

2) During interview, staff reported that on 07/08/2019, Child #1 was served and ate saltine crackers at approximately 3:00pm.

3) The licensing inspector reviewed the list of ingredients listed for the saltine crackers that Child #1 was served on 07/08/2019. The list of ingredients indicate that the crackers contain wheat.

4) Staff interviewed acknowledged Child #1 is allergic to the ingredient listed.

Plan of Correction: Per the Center: "All staff have been retrained on proper allergy prevention. All staff have been retrained on how to read an allergy list."
