Comments:
An unannounced monitoring inspection was conducted today. There are 5 children present (12 points) with one caregiver at LI arrival. A second caregiver arrived within 5 minutes.3 adult and 5 children's records were reviewed. See violation notice for further information. Children were observed playing quietly during today's inspection. Emergency escape drills, first aid supplies, fridge/freezer temperature are all in compliance. A subsidy Health and Safety Inspection (SHSI) was also conducted today.
Based on a review of records, one child's record was not maintained as up-to-date. Evidence: Child 2's record was signed 1/4/19 (past one year).

I will have the parent sign the form within the next 3 business days.

Description:
Based on a review of records, the parents have not been provided in writing with the notification of a pet in the home. Evidence: 5 children's records were reviewed. 5 of 5 were missing notification of a pet in the home. (Children 1,2,3,4,5)

I will do an addendum to the children's records.

Description:
Based on statement by the caregiver, the caregiver does not speak, read or write in English. Evidence: The caregiver present at LI arrival, stated she does not speak English. (Staff A)
Plan of Correction:
I will either acquire a new assistant or have her enroll in ESL classes.
