8VAC20-790 Subsidy Program Vendor Requirements for Family Day Homes
Comments:
An unannounced subsidy inspection was conducted on 10/19/22. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program, and were discussed during the exit interview..
Based on record review and interview it was determined that provider did not ensure that each child's record contained all of the required information. Evidence: 1. The record for child 5 did not contain proof of identification. 2. The provider confirmed this information was not available during this inspection.
Plan of Correction:
Provider will request documentation from parent at pick up.
