Comments:
Licensing Inspector conducted a safety health and safety inspection in conjunction with a voluntary registration renewal on 10/19/2022. At the time of the inspection, there were 4 children in care. All records were reviewed. The requirements in "Attachment A" of the Vendor Agreement were reviewed. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program, and were discussed during the exit interview.
Based on record review and interview it was determined the provider did not ensure compliance with all items on the Voluntary Registration health and safety checklist. Evidence: 1. The records for child #1 and #2 did not contain documentation of a physical exam. 2. The provider confirmed these records were not available during this inspection.
Plan of Correction:
Provider will request documentation from parents at pick up.
