An announced inspection was conducted at the center on Monday, September 23, 2019 from 1:15pm to 1:50pm. The center?s director was present and assisted the inspector. Two board members? background checks were reviewed on this date.

Based on a review of records and interview on 09/23/2019, the center did not ensure the results of a fingerprint based background check were obtained for two board members, hired prior to January 22, 2018, by September 30, 2018.
EVIDENCE: 1) The record for Board Member #1 did not contain the results of a fingerprint based background check. 2) The record for Board Member #2 did not contain the results of a fingerprint based background check. 3) During interview, the director reported the fingerprint results have not been obtained for Board Member #1 or for Board Member #2.
Plan of Correction:
Per the Director: The appointments will be scheduled between 9/23 and 9/24. The results will be forwarded to the licensing inspector when they are received.
