Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A subsidy health and safety inspection was initiated on 05/12/2020 and concluded on 05/13/2020. The director was contacted by telephone to initiate the inspection. There were seven children present and two staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.

The information gathered during the inspection determined non-compliance with subsidy regulations, and a violation was documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-665-520-B-11
Description: Based on a review of documentation and interview, the vendor did not obtain a written allergy care plan with the required information for one child with a diagnosed food allergy.

Evidence: 1) The written allergy care plan in the record for Child #1, who has a diagnosed food allergy, does not contain instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction. 2) During interview, a member of management confirmed the center does not have a written allergy care plan that includes instructions from a physician for Child #1.

Plan of Correction: Per the Vendor: "Child is a drop in student. Mother will bring allergy plan signed by a physician once child returns to the center."
