Technical Assistance:
Hand washing practices prior to the children eating were discussed with the program director. Hand washing is to occur immediately prior to eating.
Comments:
An unannounced,mandated monitoring inspection was conducted today. The inspector arrived at 11>40 AM and departed at 2:15 PM. Eight classroom plus the STEM lab and music rooms were inspected. Morning activities, outdoor play, lunch service and preparation for nap time were observed. Emergency contact information for staff was reviewed. A sample size of nine children's records were reviewed. Medication administration and injury records were reviewed.
An exit meeting was conducted with the program director prior to closure of the inspection.
Based upon review of nine children's records and staff interviews, the facility has not ensured that each child's record includes the names, addresses and telephone numbers for two designated persons to be contacted in an emergency when a parent cannot be reached. Evidence: 1. The record provided for child 5 does not include the street address for one of the two designated contact persons. 2. Staff 1 verified that the address was not listed for one of the emergency contact persons for child 5.
Plan of Correction:
The facility responded with the following: The parent will be contacted today to provide the required street address. Children's records will be checked to ensure that all required information is included in each record.
