Technical Assistance:
The water temperature in both rooms 5 and 7 reached and were recorded as 120 degrees F. This is the maximum temperature allowed in children's hand washing sinks and must be carefully monitored to ensure that it does not surpass 120 degrees.

Comments:
A monitoring inspection was conducted on 03/30/2022. There were 71 preschool aged children present with required staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based upon observation, the facility has not ensured that areas and equipment are maintained to be safe
Evidence:
The door to the sprinkler/electrical room was unlocked. Inside the room are numerous electrical and other wires and machinery within reach of children.
There was also a fire extinguisher sitting on the floor of the closet.

Plan of Correction: The facility responded with the following:
All staff will be reminded of the importance of keeping this room locked at all times while children are in care. Routine checks will be conducted to ensure that the room is locked.

Standard #: 8VAC20-780-280-B
Description: Based upon observation, the facility has not ensured that hazardous substances are kept in locked places.
Evidence:
The door to the staff restroom at the end of the hallway was unlocked during the inspection. There was a spray can of Febreeze air freshener on the sink.

Plan of Correction: The facility responded with the following:
All staff will be reminded of the importance of keeping the staff restroom locked at all times while children are in care. Routine checks will be conducted to ensure that the room is locked.

Standard #: 8VAC20-780-330-B
Description: Based upon observation and measurement, the facility has not ensured that the resilient surfacing under the "volcano" climber and the geodesic dome climber complies with minimum safety standards of the American Society for Testing Materials ASTM F1292-99 requiring the sufficient depth of the resilient surfacing throughout the entire fall zones of the equipment.
Evidence:
1. The resilient surfacing (rubber mulch) around the entrances/exits to the "volcano" climber has been pushed away from the area revealing less than 1 inch of resilient surfacing in these areas.
2. In numerous areas under the geodesic dome climber, the ground beneath the climber is revealed as the resilient surfacing (rubber mulch) has be pushed or washed away.

Plan of Correction: The facility responded with the following:
Our maintenance team will be contacted to install enough rubber mulch surfacing to create a six inch depth throughout the entire fall zones of each climbing structure. Procedures will be developed to ensure compliance with ASTM guidelines the procedures in the playground safety plan regarding the maintaining of the resilient surfacing.

Standard #: 8VAC20-780-330-C
Description: Based upon observation, the facility has not ensured that ground supports for playground equipment is covered with materials that protect children from injury.
Evidence:
The U-shaped ground supports for the "volcano" climber and the geodesic dome climber were visible and not covered by the mulch or a material to protect the children from injury. The mulch used under the structures was not covering these supports.

Plan of Correction: The facility responded with the following:
Our maintenance team will be contacted to install enough rubber mulch surfacing to sufficiently keep the ground supports covered. Procedures will be developed to ensure compliance with ASTM guidelines the procedures in the playground safety plan regarding the maintaining of the resilient surfacing.
