Comments:
The licensing inspector conducted an unannounced renewal inspection, at which play groups, activities, group time, centers, story time, personal hygiene, outdoor time, and lunch time were observed. Children were observed; and staff and children files and other documentation were reviewed. Fifty-six (56) children, aged 5 months-10 years, were found to be in care with 9 staff, and administrative staff. Please contact me if you need further assistance. Thank you for your cooperation. This inspection began at 8:45am and concluded at 11:45am.
Based on review of the infant room the facility failed to maintain compliance with this standard as required related to labeling of cribs.
Evidence:
Only 1 of the 4 cribs were labeled to identify ownership/use as required.
Plan of Correction:
The cribs will be relabeled and labels updated/replaced as needed for future and continued compliance.

Description:
Based on review the facility failed to maintain compliance with this standard as required relate to hand washing areas and diapering surfaces.
Evidence:
1. There was no hot/warm water running from the stand alone hand sink in the toddler room when checked by the inspector. 2. This sink is used for hand washing of the children and staff, and for hand washing after diapering.
3. There was no form of hot/warm water available in 1 of the 2 children bathrooms in the main building, as the hot water knob on the sink was missing.
4. There was no soap or paper towels available for hand washing in the school age building upon initial inspection.
5. The diapering pads in the toddler and 2's room had small tears in the waterproof surface, which would not allow for a sanitary/santized surface as required.
Plan of Correction:
Maintenance has been contacted to correct the issue regarding the hot/warm water. A knob had been purchased for the sink, but did not fit; it will be returned and another correct one purchased and installed. The diaper pads will be replaced so as they have no cracks and are sanitary for use and cleaning. The soap and paper towels were immediately replaced in the school age building during the inspection. Staff will be reminded to inform office of any no-working areas of equipment, and to ensure that soap and paper towels are immediately replaced when empty.
