Comments:
An unannounced mandated inspection was conducted on 7/6/2022. There were 34 children, ages 1 month -12 years 11 months and 8 staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. A total of 4 children?s records and 5 staff records were reviewed. The children were observed during breakfast, center time and active time on the playground. The inspector arrived for the inspection at 8:45am and departed the center at 11:33pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation, the center failed to ensure that equipment on the playground was clean, safe and operable. Evidence: Observed 2 pieces of broken equipment on the playground.
1. See Saw play structure was rusted thru and broken on one side causing a pinch point. Seat on the see saw was missing two bolts and seat would move out of place causing a pinch point. Two children in the toddler classroom were observed playing on the structure.
2. The roof on the playhouse structure was pressed in and cracked in the center causing a pinch point. There we no children observed in the playhouse but it was accessible to the children.

Plan of Correction: Center will ensure that the 2 broken pieces of equipment on the playground are removed.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center failed to ensure that hazardous substances, such as cleaning materials, were kept in a locked place using a safe locking method that prevents access by children.
Evidence: Cleaning materials in three rooms were not secured in a locked cabinet.
1. Glass cleaner was located in the medicine cabinet above the bathroom sink in the 2 year old room and was unlocked. The cleaner was out of reach of the children.
2. The cabinet in the 15 month to 24 month room was unlocked and had cleaning materials inside. Lysol spray was on top of the unlocked cabinet but was out of reach of the children.
3. Cleaning materials were located on top of the cabinet in the infant room. The cleaner was out of reach of the children.

Plan of Correction: Center will ensure cabinets have locks installed and all cabinets storing hazardous substances are locked using a safe locking method to prevent access by children.
