Comments:
An unannounced mandated inspection was conducted on 1/5/2023. There were 27 children, ages 7 months- 4year, 7 staff and 2 administrative staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. A total of 3 children?s records and 4 staff records were reviewed. The children were observed during center time, story time, craft time and lunch. The inspector arrived for the inspection at 10:25am and departed the center at 12:27pm.

Discussion on hazardous substances and other harmful agents.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 8VAC20-780-150-B
Description: Based on the review of three children?s records, the center failed to ensure that each physical examination and immunization records shall be signed by a physician or designee.

Evidence:
1. The start date for child #3 was 5/2/2022 and on the day of the inspection the physical examination record and immunization record was not signed by a physician or designee.

Plan of Correction: Provider will obtain a signed physical and immunization record from parent.
