A complaint was received and initiated on November 9, 2023. The investigation concluded on March 15, 2024. During the investigation the physical property inside and outside were reviewed and classroom observations were made. The information gathered during the investigation supports the allegation, therefore, the complaint is valid. Violations regarding physical plant were observed, appear on the violation notice and were discussed with administrators at the conclusion of the inspection.
Based on observation and interview the center failed to ensure areas are maintained in clean condition. Evidence: 1 - There were areas of a black substance throughout a section of removed floor tiles. a. The area was approximately six inches wide and the width of the building. 2 - The areas of black substance were in the dinning room where children eat meals and snacks. Children were observed eating in the room during the inspection. 3 - The areas of black substance are within reach of children as it was located on the floor. 4 - Staff #1 confirmed that the tile was removed and there was black substance in the area where the tile once was.
