Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on April 15, 2021 and concluded on April 16, 2021. The provider was contacted by telephone to initiate the inspection. There were seven children present and one staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed two children?s records, one caregiver record, and one household member record submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on review of children's records and provider interview, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to the child's attendance or within 30 days after the first day of attendance.
Evidence: 1. The record for child #1 (start date 5/01/19) did not have documentation of a physical examination. 2. The record for child #2 (start date 12/02/20) did not have documentation of a physical examination.
