Comments:
An unannounced subsidy monitoring inspection was conducted today, 8/12/19.
A violation notice was completed regarding the one repeat violation found during today's inspection.
The children were busy playing in each class, going outside, singing songs, eating lunch and taking a nap.
There are 26 children in care ranging in age from five months to four years old with five staff and one cook present.

Inspection began: 9:35 am ended 12:45 pm.
Based on observation the vendor failed to ensure that there is cushioning materials under playground equipment.
Evidence:
1. The playground equipment used by the children does not have a cushioning material under the swings, slides or climbing apparatus.
Plan of Correction:
The Director will have mulch purchased and placed under the playground equipment.
