Comments:
A subsidy health and safety (SHSI) inspection was completed. A code compliance inspection was also conducted on this date. Both inspections combined cover subsidy and code compliance regulations. The inspection started and ended on 1/24/24 from 10:20am until 12:55.
Violations are identified on the violation notice.
Based on record review, a child's record did not have all required information. Evidence: 1. Child #1 has a special need and there were no special care plan on file.

The director has asked the parents to get the care plan completed.
