Comments:
A mandated Renewal inspection was conducted today, 7/15/19. There were 69 children and 15 staff present during this inspection. Children were observed transitioning to chosen activities, making edible play-doh, collaborating for a play, and participating in gross motor activities in the gym. Reviewed 5 children's files, 5 staff files, medications and authorizations, attendance and physical plant. Violations of the Standards are recorded on the Violation Notice. Based on children's record review, the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence:

Description:
Based on children's record review, long-term prescription drug authorization did not contain written authorization from the child's physician and parent.

Child#1 (age 6) medication authorization for Benedryl and Epi-Pen Jr were expired. 5/21/18

Ill give mom the form tonight and have her take it to the doctors to update.
