Comments:
An unannounced monitoring inspection was conducted between the hours of 10:00 am and 12:30 pm. A sample of 3 staff and 5 children's records were reviewed, in addition to 3 medications. Children were observed during lunch and preparing for nap. Infants were observed sleeping in cribs, on the floor with staff and eating.
Based on observation the center did not ensure all required information was kept for each staff record. Evidence: Staff #1 (hired 10/11/19) did not have documentation of an emergency contact.
Plan of Correction:
The following corrective action was provided by the director during the exit interview: The staff documented an emergency contact in the file during the inspection. 3d

Description:
Based on observation the center did not ensure all requirements were met when diapering. Evidence: A staff in the infant room was observed changing a diaper and did not wash her hands afterwards.
Plan of Correction:
The following corrective action was provided by the director. Evidence: The director spoke to the staff during the inspection. Hand washing will be reviewed with all staff.
