Comments:
An unannounced monitoring inspection was conducted on 02/26/2020 from 12:39 pm - 1:35 pm in order to follow-up on the violations listed in the Special Order issued on 11/01/2020. At the time of entrance there were a total of 27 children in care with seven teaching staff and two administrative staff. The children ranged in age from infant - three years. Children were observed at the conclusion of lunch and preparing for nap. The outdoor play area and the indoor physical plant was reviewed in addition to a sample size of ten injury reports, the playground safety plan and the injury prevention plan for the facility. Two violations were observed and are listed on the violation notice. The areas of noncompliance were reviewed with administrative staff at the conclusion of the inspection.
Based on observation, the center failed to ensure that outside areas of the facility shall be maintained in a safe condition.
Evidence: 1. There was an area of cracked and sharp plastic on the outdoor play structure. a. One piece of the plastic fell off when touched by the inspector. 2. Cracked and sharp plastic is a safety hazard to children's fingers and hands.
Plan of Correction:
The outdoor play structure was discarded during the inspection.

Description:
Based on observation, the center failed to ensure that a copy of any special order issued by the department shall be posted in a prominent place at each public entrance of the licensed premises to advise consumers of serious or persistent violations.
Evidence: The special order issued by the Department of Social Services on 11/01/2019 was not posted.

The special order was posted during the inspection.
