Comments:
An unannounced code compliance inspection was conducted on 01/22/2024 from 12:00 p.m. to 12:50 p.m. There were 17 children present, ranging in ages from three to five years old with two staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, procedures, and medication. A total of five children?s records and five staff?s records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on review of records and interview, the center failed to ensure that the criminal history background check is received before the first day of employment. Evidence: 1. Staff #2?s start date was 09/18/2023. The criminal history background check on file for Staff #2 was dated 12/13/2023. Staff #5 confirmed that Staff #2?s criminal history background check was not received before Staff #2?s first day of employment. 2. Staff #3?s start date was 08/24/2023. The criminal history background check on file for Staff #3 was dated 08/30/2023. Staff #5 confirmed that Staff #3?s criminal history background check was not received before Staff #2?s first day of employment.
Plan of Correction:
As stated during interview, also with disclosed emails regarding VSP & DSS, inquires when checks were going to be completed. VSP took 3 mos to complete check. All employees have completed background checks completed.
