Comments:
An unannounced Renewal inspection was conducted from 11:20am-1:50pm. There were 59 children in the direct care and supervision of 12 staff members. A complete inspection of the facility was conducted. A sample size of 5 staff records, 6 children's records, 1 emergency medication, 2 diaper ointments, evacuation drills, emergency materials, and the first aid kit were reviewed. Based on a review of children's records, 1 out of 6 records was incomplete. Evidence: 1. Child #5 was missing 2 of 2 required addresses for emergency contacts.

Description:
Based on observation, areas and equipment of the center were not maintained in a clean, safe and operable condition. Evidence: 1. The toilet paper holder in the Women's restroom has one side broken off completely and will no longer hold the toilet paper roll on it.

We will install a new one. For now, we put toilet paper in a basket for the children.
