Technical Assistance:
Discussed annual training requirement for new staff.
Comments:
An unannounced monitoring inspection was conducted this date. A total of 12 children (25pts) with Provider and 1 Assistant. When the Inspector arrived, the children were cleaning up legos and other toys and equipment from morning play. Hand washing and bathroom procedures were observed. The children transitioned to lunch where there were two tables and staff positioned at each table to closely supervise and provide second helpings. Lunch today was macaroni and cheese, steamed broccoli and fresh cantaloupe with milk to drink. The home was decorated for the season with crafts that were made by the children. Artwork such as toilet paper roll bats and hand painted pumpkins. A total of 6 children's records and both staff records were reviewed today. Observation of the kitchen area, diapering room and programming space was made. The space has shelves along all walls with wooden puzzles, books, tactile equipment and other games and toys within reach of the children. Positive interaction observed between staff and children. No medications at this time and no identified food allergies currently. This inspection began at approximately 11am and ended at 1:30pm. Thank you for your cooperation today. Based on child record review, a physical exam was not obtained for one child within 30 days of the first day of attendance. Evidence: Child #1 began on 4/24/29, results of the physical exam was dated 7/17/19.
Plan of Correction:
I will reiterate the importance of obtaining the health exam within one month of each child starting.

Based on staff record review, one staff record did not maintain all required information. Evidence: Staff #2's record did not contain a second reference check.

Description:
Based on child record review, documentation of immunizations were not obtained prior to attending care for two children. Evidence: Child #1 started on 4/24/19, documentation of immunizations were obtained on 7/17/19. Child #5 started on 7/1/19, documentation of immunizations were obtained on 7/26/19.
Plan of Correction:
I will not allow a child to begin care without immunization documentation.
