Comments:
An unannounced monitoring inspection was completed on 10/28/2019. There were one child in care with two staff present. The inspector arrived at 1:40 P.M.. and departed at 4:00 PM. Five children's records and four staff's records were reviewed during the inspection.
Observed the children during: free play, snack,outside time, arrival into the program from a local school and hand washing procedures.
Discussion with the staff about lead teacher qualifications, rearranging the outside play equipment, and subsidy requirements.
Based on review of four staff's records, the center failed to ensure that all components were obtained as required by the standard.
Evidence: 1. Staff # 2, staff # 3 and 4 was missing one of the two required references.
Plan of Correction:
The director will work on obtaining the missing references and will make sure all records are complete.

Description:
Based on review of the emergency records, the center failed to ensure that a fire drills were documented.
Evidence: 1. July and September 2019 did not have a documented fire drill.
Plan of Correction:
The staff stated that a drill was conducted but was not documented. The director and staff will ensure that all drills are documented on the form.
