Comments:
A monitoring inspection was initiated on 9/10/2021 and concluded on 09/15/2021. The director was contacted by telephone to initiate the inspection. There were 8 children present and 2 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on review of two staff's records, the facility failed to ensure that all staff had the repeat TB screening/testing every two years.
Evidence:
1. Staff # 1 most recent TB was from 5/22/2019. The requirement is for the TB screening/testing to be completed every two years.
Plan of Correction:
The director will have the staff person to obtain a new TB screening as soon as possible. The director will also monitor all dates.

Description:
Based on review of two staff's records, the facility failed to ensure that all repeat CPS background checks were completed every five years.
Evidence: 1. Staff # 1 most recent CPS background check was completed on 3/16/16. The background check is required to be repeated every five years.

The director will have the staff person to submit a new CPS check as soon as possible. The director will also monitor all dates.
