Comments:
An unannounced monitoring inspection was conducted on 3/12/2024 from 11:55 am to 1:16 pm. There were two children in care, both age three, in one group with two staff. three child records, two staff records, and other documents were reviewed. Children were observed in having lunch and napping.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on record review, the center failed to obtain a physical examination for a child within 30 days of beginning attendance.
EVIDENCE: Child 2 has a documented start date of 11/28/2023. A physical form in the record has no name on it to show if it is for Child 2 or for someone else.

Ensure Children have physical exams within 30 days.

Based on observation, the center failed to ensure that cots used by children during the rest period had both a top and bottom cover or a one-piece covering which is open on three edges.
EVIDENCE: Children 2 and 4 were observed napping and the cots had no bottom covers. Staff 3 and 4 were not aware that bottom covers were needed.
Plan of Correction:
Make sure cots are covered top & bottom.
