Comments:
An unannounced complaint investigation was initiated on August 16, 2019 in response to allegations received by the Division on July 29, 2019 regarding background checks, ratios, lack of food, and volunteers. Based on observation and staff interviews, the preponderance of evidence gathered during the investigation does not support the allegations. Based on observation, the facility failed to maintain all areas inside the center in a safe and operable condition. Evidence: the crib designated as the emergency evacuation crib has a broken wheel due to missing bolts and is unable to be wheeled out of the building in an emergency.
