Technical Assistance:
Discussion on hand washing and toilet procedures, equipment and materials.
Discussed submitting correct version of the renewal application prior to expiration date.
Comments:
An unannounced renewal inspection was conducted today from approximately 10:45 am to 12:45 pm. There were 28 children supervised by 6 direct care staff. The physical plant, 3 staff records, 5 children?s records, children's nutrition, evacuation drills, injury reports, emergency supplies were inspected. The playground and surfacing were inspected. The children were observed in free play and outside play. Diaper changing and hand washing were observed. The interactions between the children and staff were seen to be positive and constructive in nature. There were age appropriate materials for all children. Staff-to-child ratios were in compliance.

Based on observation, the center did not ensure the diapering surface used only for diapering or cleaning children, and it was cleaned with soap and at least room temperature water and sanitized after each use. Evidence: Staff #3 changed a child and did not clean the diapering surface with soap after changing a child's diaper.
Plan of Correction:
Will retrain staff to ensure proper steps are taken.
