Comments:
An unannounced monitoring inspection was conducted today between 9:00 am and 12:30 pm. There were 43 children present with 12 staff members. The children were observed playing with floor toys, hand washing, eating snack and during a fire drill. The physical plant, children's records, staff records, evacuation drill log, attendance logs, emergency plans, snack menu's and emergency supplies and medication's were inspected. Based on a review of childrens' records, the center failed to ensure that children's records are up to date.
Evidence: Child #3's record did not include a phone number for the second emergency contact.

Description:
Based on observation the center failed to ensure that hazardous substances shall be kept in a locked place using a safe locking method.
Evidence: 1) In the 2's classroom, the bleach/water solution was not in a locked place. 2) In the 4's classroom, bleach wipes and a bleach/water solution were not in a locked place.

We will find a safe way to lock these items

Description:
Based on a review of evacuation log, the center failed to ensure that a monthly practice fire drill and a minimum of two shelter in place drills were conducted annually.
Evidence: The center only had documentation of one shelter in place drill.

We will conduct all required drills.
