Comments:
A mandated Renewal inspection was conducted today. There were 6 children present (12 points) with 1 caregiver. Reviewed 2 children's files, 2 staff files, emergency plan and emergency evacuation log, attendance, menus, medications and authorizations, training hours and physical plant. Based on record review, prescription emergency medications did not have written authorization from physician with instructions for dosage, frequency and amount for allergies. Evidence: Child#1 did not have physician authorization or instructions for emergency Epi pen.

I will have the parent's get the authorization and other information.

Description:
Based on record review, caregivers did not complete 16 hours of annual training. Evidence: Caregiver#2 was missing 6 hours of annual training.
Plan of Correction:
5 hours will be completed this weekend and the last hour by 12/24/19
