Comments:
An unannounced monitoring inspection was conducted today from 7:00am-8:20am. There were 35 children directly supervised by 5 staff. The physical plant, programming, menus, 5 staff records, 7 children?s records, 13 medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. All staff on site are current in CPR/First Aid training, 3 staff are trained in Daily Health Observation, and 2 staff are trained in Medication Administration Training. Children were observed playing basketball, coloring, playing games, and participating in various other table top activities. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of age appropriate materials for the children and the center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-510-N
Description: Based on review of 13 medications, it was determined not all procedures in regards to expired medication authorization forms were followed.

Evidence:
1 of 13 medications reviewed have an expired medication authorization form with the medication still on site in the medication box and was not returned to the parent within 14 days of the authorization expiration. Child #8 has an over the counter medication accompanied by an authorization form with effective dates of 9/14/18-9/14/19.

Plan of Correction: I will return the medication to the parent.
