Comments:
An unannounced monitoring inspection was conducted today from 3:40pm-6:00pm. There were 43 children directly supervised by 5 staff. The physical plant, programming, menus, 5 staff records, 9 children's records, 6 medications, emergency practice drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group gross motor exercise by running sprints and playing kickball, coloring, reading books, completing homework, and listening to story time. There was an abundant supply of age appropriate materials for the children and the center was clean and organized. All staff on site are current in CPR and First Aid training and adequate staff on site are current in Medication Administration Training and Daily Health Observation. Based on record review, 2 of 9 children's records reviewed did not contain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence: Child #3 and Child #4 did not have immunizations on file.
Plan of Correction:
I will talk to the parents and ask them to bring the immunization record as soon as possible.
