Comments:
An unannounced monitoring inspection was conducted with the director beginning at 6:50 AM and ending at 8:05 AM. There were eleven school-aged children in the care of two staff members. The children's ages ranged between five and nine years old. The children were observed playing games while listening to music. Children were also observed making bracelets. Ten children's records were reviewed. Two staff records were reviewed and in compliance. One medication was inspected and compliance. The first aid kit, flashlight, and weather-band radio were observed and in compliance. The most recent emergency evacuation drill was documented as 8/26/19. The most recent shelter-in-place drill was documented as 4/26/19. The most recent health inspection was dated 10/29/18. The most recent fire inspection was dated 11/20/18.

Based on the review of ten children's records, it was determined that not all of the children's records were complete. Evidence: Child A's record was missing a parent's work address and phone number and the name, address, and phone number for a second emergency contact. Child B's record was missing the name, address, and phone number for a second emergency contact.
Plan of Correction:
The director stated that she would talk to the parents and request the missing information during pick up this afternoon.
