Comments:
An unannounced monitoring inspection was conducted on 11/22/2022 at 3:35pm. There were 20 (school-agers) directly supervised by 2 staff. The physical plant, 2 staff records, 5 children?s records, 1 child's medication and medication authorization record, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in individual and group activities and preparing for a pizza party. Hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Based on review, the facility did not ensure that children's records were maintained with the required information. Evidence: reviewed 5 children's records and found that Child 2 did not have complete contact information for an emergency contact; Child 3 did not have a second emergency contact; Child 5 did not have an second emergency contact.
