Comments:
An unannounced Monitoring Inspection was conducted today from 4:28pm to 5:40pm. There were 34 school-agers directly supervised by 4 staff. The physical plant, 5 staff records, 5 children?s records, 6 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, playing outside, eating snack and socializing. Hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Standard #: 22VAC40-185-60-A
Description: Based on review, the facility did not ensure that children's records were maintained with the required information. Evidence: Reviewed 5 children's records and found that 3 did not have documentation of both parent's contact and employment information.

Plan of Correction: Will reach out to the parents today.

Standard #: 22VAC40-185-240-C
Description: Based on review, the facility did not ensure that staff who work directly with children shall annually attend 16 hours of staff development. Evidence: Reviewed 5 staff records and found that 1, Staff 5 did not have documentation of 16 hours of annual staff development.

Plan of Correction: Corrected at the time of the visit.
