Comments:
An unannounced monitoring inspection was conducted today with the director, who was present with 3 additional staff and 22 children, ages ranging from 5 to 10 years. The inspection took place from 7:25 am to 8:15 am. The children were engaged in tabletop games and then transitioned to 3 stations set up for physical activities. The children eagerly recited the rules and expectations of the program and then recited positive affirmations to include bravery, encouragement, trustworthy and safety. There were no medications on site. Two children had diagnosed food allergies. One injury/accident report was reviewed during this inspection. Five staff files and six children's files were reviewed. The areas of non compliance are outlined on the violation notice.

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on a review of children's files and an interview with the director, it was determined that one child ( A) did not have a record on site.

Plan of Correction: We will locate or establish a new file for child A.

Standard #: 8VAC20-780-60-A-8
Description: Based on a review of six children's files and interview with the director, it was determined that two children did not have allergy care plans on file for their diagnosed food allergies.
Child A has an egg allergy and child B has a egg yolk allergy. Neither child had an allergy care plan from a physician on file.

Plan of Correction: We will obtain allergy care plans from the parents.
