A file review of staff files was conducted at AlphaBEST's main office in Woodbridge on 1/22/2020 with the assistance of Sharon Allen, Shawanda Henderson, Keesha Minor and Cathy Aylor. Based on review, the facility did not ensure that they obtained a copy of the results of a search of the central registry maintained pursuant to ? 63.2-1515 and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years for any founded complaint of child abuse or neglect against them. Evidence: Reviewed 5 staff records and found that Staff 1 did not have documentation of obtaining results of the central registry check.
Plan of Correction:
A copy of the central registry findings will be obtained by human resources.
