Written documentation that the individual possess the education and experience required by the job position also needs to be included in all staff records.

A file review of staff files was conducted at AlphaBEST's main office in Woodbridge on 1/22/2020 with the assistance of Sharon Allen, Shawanda Henderson, Keesha Minor and Cathy Aylor. Based on review, the facility did not ensure that they obtained a copy of the results of a search of the central registry maintained and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided int he preceding five years for any founded complaint of child abuse or neglect against them.
