Comments:
An unannounced monitoring inspection was conducted from 2:00 pm - 3:00 pm. There were 37 children in care being supervised by 5 staff. The children were observed participating in structured center activities. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 5 staff records were reviewed.

Based on review of three children's records it was determined that not all records contained documentation of an immunization record by the first day of enrollment. Evidence: Child A (start date: 05/16/22) did not have documentation of an immunization record on file.

Administration will request an up to date immunization record from the parent of Child A.

Based on review of medication it was determined that not all medication was picked up by a parent within 14 days when a medication authorization expires. Evidence: The authorization form for the emergency medication for child A expired on 10/03/2022.

Administration will request that the parent of child A and physician complete a new medication authorization form.
