Comments:
An unannounced monitoring inspection was conducted from 4:30 pm - 5:15 pm. There were 18 children in care being supervised by 3 staff. The children were observed eating snack and preparing to go outside. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and three staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of five children's records it was determined that not all records contained documentation of immunizations before the child's first day of attendance. Evidence: The record for child A (start date: 08/22/23) did not contain documentation of immunizations.

Administration will request an up to date immunization record from the parents of child A.
