Comments:
An unannounced monitoring inspection was conducted on 10/24/2023 from 4:45pm to 5:55pm. There were 34 children ages 5 years old to age 11 years old supervised by 6 staff. The physical plant, outdoor area, programming, attendance, menus, 6 staff records, 4 children?s records, 7 medications with corresponding authorization records, emergency drills, emergency supplies, and policies were reviewed. Children were observed participating in outside play, transitioning to indoor activities, and games. Handwashing procedures were also observed. There was an adequate number of staff present with current certification in Medication Administration Training (MAT), CPR and First Aid, and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice.

Based on review of the written attendance record and interview with staff, the center did not ensure they maintained a written record of daily attendance for each group of children documenting the arrival and departure of each child in care as it occurs.
Evidence: During inspection, there were 34 children present, but the written record reflected there were 40 children present.
