8VAC20-780-560G All food from home, including water bottles if filled at home, will need to be labeled with name and date.

Comments:
An unannounced renewal inspection was conducted on 04/10/2023 from 10:45am to 12:10pm. There were 10 children ages 5 years to age 9 years old supervised by 5 staff. The physical plant, programming, attendance, menus, 3 staff records, 3 children?s records, 2 medications with corresponding authorization records, emergency drills, emergency supplies, and policies were reviewed. Children were observed participating in legos, games, and art activities. Handwashing procedures were also observed. There was an adequate number of staff present with current certification in Medication Administration Training (MAT), CPR and First Aid, and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice.

Description: Based on review of 2 medications and medication authorizations, the program did not ensure they follow the policy for administration of medication.

Evidence:
For 2 medications for Child #4, the parent?s authorization was not given for both long term medications onsite.

Plan of Correction: Obtain missing signatures from parent.
