Comments:
An unannounced monitoring inspection was conducted from 7:00 a.m. - 8:30 a.m. There were 20 children in care being supervised by two staff members. The children were observed participating in structured center play, table top activities, and cooperative games. Breakfast served today is ham and cheese croissant, cereal bar, applesauce, and milk. Six children's records were reviewed. Three staff records were reviewed and in compliance. Two medications were reviewed and in compliance. Based on review of six children's records it was determined that not all records contained documentation of immunizations before the first day of attendance. Evidence: Child A and Child B with a start date of 08/26/2019 did not have documentation of immunizations on file.
Plan of Correction:
Administration will request a copy of the immunizations from the parents of Child A and Child B.
