Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A renewal inspection was initiated and concluded on 10/12/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 39 children present, ranging in ages from 5 years to 12 years, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on review of three staff records it was determined that staff records did not always contain documentation of a tuberculosis (TB) screening every two years. Evidence: The most recent TB screening on file for staff A was dated 07/29/2019.
Plan of Correction:
Staff A will obtain a TB screening and submit the results to administration.

Description:
Based on review of three staff records it was determined that administration failed to contact the office of background investigation to request the status of a central registry check when the background check was not returned within the first 30 days of employment. Evidence: The record for staff B (start date: 08/25/2021) did not have documentation of a central registry check nor documentation that the status of the background check had been requested.
Plan of Correction:
Administration followed up on the status of the central registry check on 10/12/2021.
